Amsler Grid Test

Abstract

The Amsler grid demonstrates particular utility in detecting early manifestations of age-related macular degeneration (AMD), central serous chorioretinopathy, and other maculopathies by identifying metamorphopsia, scotomas, and other central vision abnormalities. While the test shows high sensitivity for moderate to severe macular changes (sensitivity 78.5%, specificity 85.6%), its effectiveness is notably dependent on patient compliance and proper administration.

Instructions

  1. Position yourself at a normal reading distance from the screen

  2. If you wear reading glasses, put them on

  3. Cover one eye

  4. Focus on the central dot

  5. While keeping your focus on the dot, notice if:

    • Any lines appear wavy or distorted

    • Any areas appear blurry or missing

    • All corners and sides of the grid are visible

  6. Repeat the test with your other eye

Important: This is for educational purposes only. If you notice any distortions or changes in your vision, consult an eye care professional immediately.

The Amsler grid is a fundamental diagnostic tool used to detect and monitor changes in central vision, particularly for conditions affecting the macula such as:
  • Age-related macular degeneration (AMD)

  • Macular edema

  • Central serous chorioretinopathy

Key points about the Amsler grid test:

  1. Purpose:

  • Early detection of vision changes

  • Regular monitoring of known conditions

  • Self-monitoring between professional eye exams

  1. What it detects:

  • Metamorphopsia (distorted vision)

  • Scotomas (blind spots)

  • Central vision defects

  1. Advantages:

  • Non-invasive

  • Quick to administer

  • Can be done at home

  • Sensitive to early changes

  1. Limitations:

  • Only tests central 10° of vision

  • Subjective test

  • Requires patient cooperation and understanding

  • Not a replacement for comprehensive eye exams
